Two questionnaires issued by the NZO's Aliyah and Intelligence Department during the Second World War. Within them are filled in handwriting the details of two members of the Blak family from Poland (today Ukraine), by Dr. Shimshon Unichman. On some of the questionnaires are attached photographs of the described person.
